THE Hartlepool Mail Best of Health Awards 2012 were launched three years ago to publicly honour those in the health care sector who work tirelessly for the benefit of others and whose care and compassion has helped change lives.
We want you to tell us about those special people who have either touched your life or the lives of your family and friends.
Our categories cover every aspect of health care and take in doctors, nurses, GP and dental practices, opticians, hospital teams, community and voluntary workers and those with long and distinguished service.
The awards are being backed by both NHS Hartlepool and North Tees and Hartlepool NHS Foundation Trust, local health companies IntraHealth and Hart Biologicals as well as Hartlepool College of Further Education, Cleveland Fire Support Network and The Academy at Shotton Hall.
Nomination forms can be downloaded at www.hartlepoolmail.co.uk then filled in and emailed to us.
Paper copies are also available in the reception areas at the University Hospital of Hartlepool, in Holdforth Road, One Life Hartlepool, in Park Road, as well as the Mail reception.
The forms can completed and sent to us at Hartlepool Mail, New Clarence House, Wesley Square, Hartlepool, TS24 8BX.
The deadline for nominations is Sunday, June 3.

A judging panel will meet in early June and the winners will be announced at a plush black-tie evening ceremony at Hardwick Hall, in Sedgefield a few weeks later.
