Conference to hear of health scheme success
A NOVEL partnership scheme trialled in Hartlepool will be showcased at a national conference attended by top politicians and health bosses.
Social care organisation Turning Point, which provides specialist and integrated services for people with varying needs, was pivotal to a national pilot scheme launched in the Owton Ward area of the town.
The scheme called Connected Care is a link-up between health, social care and housing services to offer integrated personalised care and support in communities and is backed by award-winning Intrahealth which has a contract with NHS Hartlepool to provide GP services in the town.
Elizabeth Carroll, of IntraHealth, will address the high-profile conference in October, at the Birmingham Hilton Metropole, after she appeared in the Mail speaking about the benefits of the linked-up partnership approach.
IntraHealth, which runs practices at Hartfields and in Wynyard Road, Hartlepool, is a central partner in the pioneering Turning Point scheme.
Mrs Carroll will tell the audience about the project which recently featured in the Mail and the benefits of working with Turning Point and Connected Care.
We told how the Government's Connected Care initiative, which was set up in the Owton Ward area of Hartlepool, has proved a huge success to residents.
Mrs Carroll, patient and liaison officer for IntraHealth, said: "The partnership that has developed between Connected Care and IntraHealth in Hartlepool is leading the field in community engagement, contributing to shaping local health care provision to meet the needs of its patient group".
Key speakers at the event include Health Secretary Andrew Lansley, NHS chief executive Sir David Nicholson and Stephen Dorrell, former Health Secretary and chair of the Health Select Committee.
Hilary Samson-Barry, director of statutory relations for Turning Point, is part of a team developing Connected Care across the country and is keen for the conference to hear the national story of Connected Care alongside the local delivery and experience in Hartlepool.
She said: "It is particularly pertinent, now that the coalition government has given a commitment to devolve budgets to groups of GPs, to support new provider models, including social enterprise and strengthen accountability of public services to local communities."
Dr David Anderson, IntraHealth's medical director said "Elizabeth has taken the concept of patient and public engagement to another level in Hartlepool."
The two-day conference will give an update on the key developments and challenges facing primary care and will be held on October 20 and 21.
