The toddler was laid to rest today as the community remembered him with balloons and flowers.
People in the town and further afield have been touched by the little boy's story and wanted to remember him on the day of his funeral.
A service took place at St Joseph's R.C. Church in the town, before members of the public released balloons as a carriage carrying his coffin rode by.
The sky shone yellow as the balloons were let go - and people applauded as members of Jacob's family drove by.
The two-year-old died earlier this month after spending five days in an induced coma. He choked on a grape at the Pizza Hut restaurant in Hartlepool Marina, and stopped breathing for 30 minutes.
Almost £20,000 has been raised for little Jacob and his family on Go Fund Me. If you want to donate, click here.
