Ask yourself who has impressed you with their drive and determination to make a difference.

They might be someone whose campaigning for better health services, a brilliant fundraiser or a heart patient who has inspired others by overcoming daily challenges.

Heart disease continues to devastate the lives of millions of people.

But in every community there are individuals who are presently striving to change that, and the heart hero awards exist to celebrate their work.
